Mission Statement

Our mission is to provide guidance for a continued maintenance of quality diabetes self-management educational patient program. Both diabetes educator and the patient will benefit from this support and measures in terms of structure process and outcomes will be defined.

Program Coordinator Job Description

The coordinator for the diabetes self-management education program is a health professional who is a registered nurse and is an experienced health care professional with a strong background in diabetes education. Certified by the National Certification Board for Diabetes Education is desirable. Experience in a managerial or supervisory role capacity is required. The program coordinator reports to the Director of Health Education, John O'Shea, M.D.

Responsibilities:

  1. Serve as a liaison for the program staff, advisory committee and facility administration.
  2. Provide and/or coordinate the orientation and continuing education for the program personnel.
  3. Provide assessment, teaching and follow-up instruction to program participants.

Goal Achievement of DSME Operations

Programs goal is to provide guidance to the person having diabetes in learning self management skills and making lifestyle changes with a goal to either avoid or delay the complications associated with this disorder.
